On the Establishment of “Doctors for Future”
A Healthcare Professionals’ Association for a Sustainable Society
Environmental changes in the world we live in — including climate change, marine pollution, and the loss of biodiversity — are already exerting profound impacts on human health, daily life, and economic activity.
It has moved far beyond mere speculation that these changes are largely driven by human activity; this has been scientifically demonstrated and verified through extensive research and accumulated evidence.
It is a profound contradiction that various socioeconomic activities, including healthcare — which should fundamentally enrich and protect human life — are now threatening our very existence.
To turn away from this reality would be tantamount to abandoning our own future.
Unprecedented climatic conditions, plastic pollution that damages ecosystems, and the spread of emerging infectious diseases all place healthy and culturally fulfilling lives at risk. Addressing these challenges is fundamentally aligned with the very mission of medicine and healthcare, which have evolved through the long history of scientific inquiry.
As a gastrointestinal surgeon, I currently perform more than 200 operations under general anesthesia each year. What I strongly perceive in medical practice, especially in surgery, is the enormous amount of waste and environmental burden involved.
Under the justification of “medical necessity,” our field has long operated with values somewhat detached from those of broader society, resulting in excessive consumption and environmental impact. The vast quantities of disposable materials discarded after every operation, as well as surplus pharmaceuticals including antibiotics, could be significantly reduced through greater awareness and modest changes in practice.
By paying attention to the environment of the communities in which we live and imagining the severe damage occurring across the wider world, we can foster the values and awareness needed for younger generations to build a sustainable society. I firmly believe that taking responsibility for protecting the oceans and land where we live is fundamentally inseparable from our responsibility toward the families and patients we care for.
Those who already hold a certain degree of social standing, in particular, have a responsibility to contribute to society. This is precisely the spirit of noblesse oblige.

持続可能な社会を考える医療者の会 ( Doctors for Future )設立にあたって
気候変動、海洋汚染、生命多様性の消失といった我々の暮らす世界の環境変化はすでに人類の健康な生活や経済活動に甚大な影響を与えており、こうした変化は人類の活動によるものが大きいことが既に疑いの段階を過ぎ、科学的に様々な知見として証明、検証がなされております。
本来人類を豊かにするはずである医療を含む様々な社会経済活動が、私たちの存在すら脅かしつつあることは大きな矛盾であり、こうした現実から目を背けることは、私たち自身の未来を放棄することと同義でしょう。
人類が過去に経験したことのない気候や、生態を蝕むプラスチック汚染、新興感染症の蔓延などは健康で文化的な生活を危機に陥れる要因であり、こうした問題に取り組むことは長い科学の歴史で培われた医学・医療の目的そのものと同一です。
私は現在消化器外科医として年間２００例以上の全身麻酔手術を執刀しています。手術を含む医療の現場でまず感じることは、とにかく多くの無駄と、環境への負荷です。これまで社会は「医療」という免罪符の言葉とともに、我々の領域では一般社会とやや異なった価値観のもとで大量消費、環境負荷が行われてきました。毎回の手術の際に廃棄される大量の消耗品、抗菌薬を含む余剰な医薬品はちょっとした心がけで削減できる余地が十分にあります。
自分たちの住む街の環境に目を向け、大きな世界に起こる甚大な被害を想像することで、持続可能な社会を担う若い心が育まれます。そして自分の住む海と大地の保全に責任を持つことは、自分が支える家族や患者さん達への責任と本質的には同一であると信じております。
既に一定の社会的地位にある者達こそ、社会に対して貢献する責務があると考えております。それこそノブレス・オブリージュ（ noblesse oblige
）の精神です。
