Small Steps Toward Green Surgery: Becoming Surgeons for Planetary Health

 

The global average temperature is expected to reach another historic high this year, while plastic pollution, soil degradation, and biodiversity loss have already exceeded planetary boundaries critical for human survival. Healthcare industry in Japan accounts for 5% of national greenhouse gas emissions, comparable to those of major industries. Operating rooms are particularly environmentally intensive, generating large amounts of waste and ecological harm caused by pharmaceuticals in addition to greenhouse gas emissions. Healthcare and other socioeconomic activities, originally intended to improve human well-being, are now threatening the foundations of human existence. Ignoring this reality means abandoning our future. Recognizing that healthcare professionals must first confront the global environmental crisis, we began small but practical initiatives in 2020 to reduce the environmental burden of medical practice and support local sustainability. We introduce the concept of planetary health and consider the role and responsibilities of surgeons in building a sustainable future.

 

Green surgery への小さな取り組み：Planetary health を考える外科医となるために

 

今年度も地球の平均気温は人類史上最高を記録する見込みであり、プラスチックによる海洋汚染や土壌の消失、生命多様性の消失などの各項目はすでにPlanetary boundaries と呼ばれる人類存続の限界を超えている。我が国の医療分野から排出される温室効果ガスは全産業の中で約５％を占めるとされ、鉄鋼業や運輸業、農業などの主要な産業に匹敵する。その中でも手術室が与える環境負荷は深刻とされ、温室効果ガスのみならず膨大な廃棄ゴミや薬剤による生態系への影響は危機的である。本来人類を豊かにするはずである医療を含む様々な社会経済活動が、私たちの存在すら脅かしつつあることは大きな矛盾であり、こうした現実から目を背けることは、私たち自身の未来を放棄することと同義である。医療者が変わるためにはまずは医療者自身がこの世界の現実に目を向けなければならないとの信念から、2020年より有志の医師たちによる身近な環境保全と医療界の環境負荷改善に向けた小さな取り組みがはじまった。少しずつ仲間の輪も広がりつつある。地域住民とともに持続可能な環境づくりに取り組む外科医の視点から、人の健康と地球環境の保全を包括的に捉える Planetary health の考え方を紹介し、私たち医療者のあるべき姿について考察する。
